1. Items and Methods of Collecting Personal Information
The Clinic collects only the minimum personal information necessary for service use during internet membership registration and reservations. Essential information for medical treatment is collected through medical record creation without separate consent in accordance with the Medical Service Act. For additional services (advertising) outside of medical treatment, a separate collection and use consent form must be completed. Not completing the consent form will not result in any disadvantage to medical treatment.
- Required items: Name, date of birth, gender, address, age, contact information, mobile phone, email, clinic registration number, service application status
- Health information: Visit information, disease information, prescription information, medical treatment information
- Payment information: Card company name, card number and other card payment approval information

2. Purpose of Collection and Use of Personal Information
① Required
- Medical information: Providing medical services for diagnosis and treatment, and administrative services such as billing, payment, and refund
- Reservation information: Collecting name, phone number, gender, and date of birth as required items for membership registration, identity verification, and medical appointment/inquiry services. The collected information is used as basic data for user identification and smooth service provision.

- Marketing and promotion: Delivering advertising and promotional information such as clinic news, clinic procedures and events, surveys, personalized advertising and marketing, cosmetics, etc.

3. Retention and Use Period of Personal Information and Destruction Procedures and Methods
[Retention and Use Period]
- The Clinic retains collected customer personal information only for the legal period (10 years) for patient register (5 years) and medical records, after which it is deleted from the database. However, if creditor-debtor relationships remain, retention continues until settlement.
- Immediate deletion upon request from information provider
However, even if the collection purpose or provision purpose is achieved, personal information may be retained if there is a need to preserve it according to the provisions of the Commercial Act and other laws.
* Records of consumer complaints or dispute resolution: 3 years (Consumer Protection Act in Electronic Commerce, etc.)
* Records of credit information collection/processing and use: 3 years (Credit Information Use and Protection Act)
* Records of identity verification: 6 months (Act on Promotion of Information and Communications Network Utilization and Information Protection, etc.)

[Destruction Procedures and Methods]
- Destruction procedure: Immediate destruction according to destruction methods after legal retention period
- Destruction method: Personal information stored in electronic file format is deleted using technical methods that cannot reproduce records, and personal information printed on paper is destroyed by shredding or incineration
- Upon termination of customer management service consignment: The consignee permanently destroys related files and database data within 2 weeks

4. Installation/Operation of Automatic Personal Information Collection Devices and Refusal
In addition to the above personal information, the service recognizes visitors through cookies, pixel tags or web beacons, and IP addresses and uses them for the following purposes. Using the Clinic website and services is considered consent to the use of such technologies as cookies. (Cookies are small text files sent by a website or service provider to the web browser in a user's computer, which allow the website or service provider's system to recognize the user's web browser and store and remember certain information. Pixel tags or web beacons are transparent graphic images placed on web pages or emails that indicate whether pages/images have been viewed or command browsers to display content from other servers. An IP address is online address information assigned by an internet network provider to devices such as PCs of users accessing the internet. There are very diverse views in each country regarding whether IP addresses constitute personal information.)

Cookie Setting Refusal Method
○ Example: As a method of refusing cookie settings, you can allow all cookies, go through confirmation each time cookies are saved, or refuse storage of all cookies by selecting options in the web browser you use. Setting method example (for Internet Explorer):

5. Rights of Users and Legal Representatives and How to Exercise Them
Reservations for minors are made through separate forms written in plain expressions that are easy for minors to understand, and consent from legal representatives is mandatory when collecting personal information.
To obtain consent from legal representatives, the Clinic collects minimum information such as the legal representative's name and contact information from minors and obtains consent from legal representatives according to methods specified in the Privacy Policy.
Legal representatives of minors may request access to, correction, and deletion of minors' personal information. To view, correct, or delete a minor's personal information, click on member information modification, go through legal representative verification procedures, and then the legal representative can directly view, correct, or delete the minor's personal information, or contact the Personal Information Protection Officer by mail, phone, or fax to take necessary measures.
※ Personal information that is mandatory to retain by law cannot be modified or deleted within the retention period even if requested.

6. Method of Withdrawing Consent
You may withdraw your consent to the collection, use, and provision of optional personal information at any time during online reservations. If you contact the Personal Information Protection Officer by mail, phone, or fax, we will take necessary measures such as destroying your personal information without delay.

7. Provision and Sharing of Personal Information
The Clinic does not use your personal information beyond the scope announced in the collection and use purpose or provide it to other individuals or companies/institutions under any circumstances except with your consent or in accordance with relevant laws and regulations.
- Submission of medical records to the Health Insurance Review and Assessment Service for medical expense benefit claims under the National Health Insurance Act
- Processing and providing in a form that cannot identify specific individuals for statistical compilation and academic research
- Submission when requested by investigative agencies according to procedures and methods stipulated by law, etc.

8. Provision and Consignment of Personal Information to Third Parties
The Clinic consigns personal information for internet reservation and text notification services as follows and stipulates necessary matters to ensure safe management of personal information when signing consignment contracts in accordance with relevant laws and regulations.

10. Measures to Ensure Safety of Personal Information
The Clinic has various security devices in place as technical measures to protect users' personal information. All information provided by users is safely protected/managed by security equipment such as firewalls.
In addition, as administrative measures to protect users' personal information, the Clinic has established procedures necessary for access to and management of users' personal information, minimizes personnel handling users' personal information, and conducts continuous security training. We also designate users of systems that process personal information, assign user passwords, and regularly update them.

11. Obligation to Notify Policy Changes
This Privacy Policy was enacted on October 22, 2025, and if there are additions, deletions, or modifications to the content due to changes in laws, policies, or security technologies, we will announce the reasons and contents of changes through the Clinic website at least 7 days before implementing the changed Privacy Policy.
